Question
How reliable is wrist widening clinically to think in terms of rickets and to order for x ray wrist? I happened to see two cases recently with clinical wrist widening. Thought it might be rickets. One had bow legs too. But x ray was perfectly normal. Why?
Answer
Correlate clinical findings with X ray as well as biochemical abnormalities. Infants usually have wrists that appear widened but one needs to palpate from lower ends of ulna and radius downwards and see if they flare up. Only then should one consider rickets.
