Question
What will be the peripheral smear finding in splenectomised thalessemia patients? What is the youngest age at which we can safely give ceftriaxone? What is the pathology of subcutaneous nodules?
Answer
Peripheral smear will show more abnormal microcytic hypochromic red cells with target cells and anisopoikilocytosis and nucleated RBCs.
Ceftriaxone can be given even in newborns.
Subcutaneous nodules are solid skin lesion that can be palpated as round or oval shaped. Found at any depth from epidermis to subcutaneous tissues. Can be a sign of serious systemic disease (e.g. TB, deep mycoses, rheumatoid arthritis, sarcoidosis, lymphoma and metastases). Also occur from benign (keratoacanthoma) or malignant (SCC/BCC) proliferation of keratinocytes. Other common, less serious causes are dermatofibroma, epidermoid cyst, granuloma annulare, lipoma etc.
