Answer
Rule out ambiguous genitalia and hypopitutarism. Look for other endocrine abnormalities and a consultation with an endocrinologist will be essential. Also rule out syndromic association.
For dialysis, apart from serum creatinine and Blood urea, one has to see clinically whether the child has uncontrolled hypertension, is oliguric and has signs and symptoms of uremia in which case dialysis may be required. In addition if the child hyperkalemia and severe metabolic acidosis not controlled with medical line of management, then dialysis may be indicated. For ongoing HUS activity, plasmapharesis may be required.
