Sir. I am an intern from Mumbai. Can you kindly tell me when do we start monitoring for the side effects of steroid therapy in a child of nephrotic syndrome i.e. when do we start to look out for hypertension, cataract formation, etc. It would be really helpful if you would give a time table regarding when these side effects begin to crop up after initiation of therapy.
Answer
The common side effects of steroids are : hypertension, hypergylcemia, cushing features, hepatomegaly, osteoporosis, myopathay and cataracts. Hypertension, hyperglycemia and hepatomegaly are seen within the first 4 weeks of 2mg/kg of steroids in Nephrotic syndrome. Osteoporosis, cataracts and myopathy appear later. Screening for these is required if a child is a steroid dependent or a frequently relpasing nephrotic syndrome. A yearly X-ray spine or DEXA scan, ophthal evaluation for catarct and Gowers test for myopathy are recommended.
