Question
Is it possible for the second child in a set of twins to be infected with HIV virus during birth from the Mother and the first twin to be normal? The second baby came after 5 minutes on arrival of the first baby. They were delivered normally and they both weighed 3.5 and 3.4 respectively.
Answer
Yes, it is possible if the transmission of HIV has taken at the time of delivery or while breast feeding. At the time of labour there is 13-20% chance of infection and another 20-30% risk with breast feeding. So the chance of transmission will vary in each twin. Placental transfer of HIV can occur and can affect one twin if the the twins are dizygotic whereas if monochorionic monoamniotic and intrauterine transmission through placenta can occur in both twins and can affect both twins.
