A mother of a 4 days young neonate (who was normally delivered and cried immediately) is having persistent 102 degree Celsius fever since day 2 of life. She had a Maternal history of intermediate white per vaginal discharge during ANC for which due treatment taken & no other problems where faced. On physical finding = there was no refusal of feed-taking adequate frequent breast feeds,normal voidings,no convulsion, no any abnormal features or rash. on investigations Hb-17.9,total leuko-10800,poly-67,lymp-30,eo-7,mono-2,Platelet-210000,CRP 2.6,sgpt-17.8,CREAT-1.6. on treatment with-chloroquine,taxim-sulbactum,paracetamol. Fever did not come down. kindly suggest your valuable guidance for further needful diagnostic & management purpose as soon as possible.
Answer
Rule out dehydration fever especially since creat is on the higher side and CRP and WBC are normal. Give plenty of fluids.
