An Obstetric colleague asks for help, I am providing a very sketchy history; they are an outstation couple having two 2 issues; the baby was alright till 4th.month; But both developed ascending paralysis beginning legs and expired around 12 mts. they are planning a 3rd child; suggestions please;I am stumped.
Answer
It would be very difficult to comment upon the limited history given but looks like a familial or inherited paresis. You would need to rule out congenital myopathies as well as neuronopathies such as SMA which can lead to gradual muscle paralysis and finally death. These are autosomal conditions and need to be counseled on the risk rate of 25-50%.
