Question
In the article appearing on this web site on management of the 'nephrotic syndrome', no diuretics has been advocated. Is there no role of the diuretics in nephrotic syndrome? What is management of choice in child presenting with severe anasarca with ascites? Child is on steroids already.
Answer
Diuretics are generally not given in a child with nephrotic syndrome as the child's intravascular state is contracted. Diuretics are given only if the child is not passing adequate urine i.e the urine output is below the oliguric level. In a patient with generalised anasarca and ascitis, diuretics may be useful provided the intravascular compartment is not dehydrated and the electrolytes are normal. A suitable option for massive ascitis would be to tap dry the ascitis and then to give a ringer lactate and frusemide challenge.
