Question
A 24 year old female conceived last year. First trimester sonography was normal. 4 months sonography showed mass, attached to the foetus. 5th month sonography confirmed that the mass was mostly bladder mass, of a large cyst. The patient was induced and then delivered at 5 months. Torch was negative, cord blood was examined, placenta was examined, hp was done and no significant positive finding was revealed. Now the patient wants to conceive. As her family physician, what investigations and where can i get those done to rule out chromosomal and tumor anomalies in the foetus? Please guide me.
Answer
It would be worthwile to know whether a proper autopsy was done on the fetus to know exactly what was the malformation. If the foetus had a bladder extrophy, then it is not an inherited condition and the chances of recurrence are very minimal. To prevent certain anomalies, antenatal folic acid to the mother is useful. To check for chromosomal anomalies in the fetus, amniocentesis at 12 -14 weeks of gestation or chorionic villus sampling at 6-7 weeks can be done. You need to consult a gynecologist preferably attached to a bigger hospital who does these procedures.
