Question
My daughter had strep throat 2 months ago (age 11). 1 month ago she had an onset of LUQ pain, fatigue & loss of appetite. 2 weeks ago she had an onset of L flank pain & enlarged lymph nodes under her chin. Labs (CBC, amylase, liver enzymes, sed rate) showed normal values. Cat Scan of abdomen was unremarkable. She also experienced constipation over past week which got resolved. Symptoms remained the same. She is unable to go to school or participate in any activities. The Doctor feels this is viral but it has been 5 weeks. Do you have any thoughts?
Answer
One needs to rule out conditions such as urinary tract infections in view of left flank pain. Also infectious mononucleosis should be kept in mind in view of neck lymph nodes. Finally streptococcal infection may lead to Rheumatic fever or Glomerulonephritis.Consult your pediatrician for the same. If still no clue, a lymph node biopsy would be useful.
