Two year old female, known to have a case of PURE RED CELL APLASIA was receiving blood transfusion since birth. His blood group was O +ve confirmed at many places including tertiary care centers. Last time when his blood group it was found to be B +ve and he was even transfused B +ve blood. Blood group was reconfirmed and found to be B +ve. What could be explanation?
Answer
First and foremost get the blood group rechecked. If it is confirmed to be the same, it may be due to the possibility of immature blood group antigen formation on the patient's RBCs which can be seen in the neonatal period as well as upto 4 months of age. Thus on testing with agglutination method, absence of A and B antigen may lead to a false blood group test. Hence it is advised that when transfusing a child upto 4 months of age, one must crossmatch with the mothers blood group. Baby's blood group antigens are well formed by 4 months of age after which the cross match should be done with the baby's blood group.
