Question
A female child few years old with chronic pallor for 2 years, needed blood transfusion 4 times previously. No fever , hepatosplenomegaly or lymphadenopathy is seen. Investigations: HB : 5.5 gm/dl, RBCs : 2500000, platelets : 120000/cc, wbcs : 11000, hb electrophoresis : Hb A1.
Answer
Since there is no hepatosplenomegaly and the child requires repeated transfusions, one should rule out nutritional anemia, chronic blood loss, as in worm infestation and also maybe a bone marrow examination, since 2 cell lines (HB and platelets) are low. On needs a very good CBC with all indices and reticulocyte count with peripheral smear examination to decided on further line of investigation. Other chronic infections such as HIV, Hepatitis B should also be ruled out.
