Question
A 9 year old girl was presented with pain and swelling in both knees for 15 days. Previous history of similar symptoms was present in the ankle, which settled on its own. CVS and ECG is normal.ESR is 95,aso titer is 200iu. CRP is positive. Child is put on aspirin and prednisolone assuming JRA, 99 percent improvement was seen in 2 days, child is able to walk with only slight pain....can it be rheumatic fever also.. Please guide on further management and treatment. Thanks.
Answer
Do an RA factor, ANA and ds DNA. It may be JIA but one needs to wait for 6 weeks. One may just give NSAIDS. Steroids may be withheld till diagnosis of JIA is not confirmed.
