A 13 years old girl was hospitalised one year back for pericardial effusion. She was put on anti tubercular therapy. Currently after a year also she had massive pericardial effusion with restricted ventricular function. She required therapeutic pericardial tap on regular basis. Fluid was negative for AFB, Anti DS DNA and ANA.

What is the etiology of this pericardial effusion and how should it be treated?

A 7 years old boy born to a commercial sex worker was referred by an orphanage for HIV testing. His HIV ELISA at time of admission to the orphanage 2 years ago was negative. Currently the child was on antituberculous treatment for past 3 months in view of primary complex. His HIV current ELISA now was positive. He had never received any blood products.

How did this child's HIV ELISA change from negative to positive?

Last month's teaching file: An 18 months old HIV infected boy on antituberculous therapy {ATT} had persistent thrombocytopenia. His bone marrow aspiration done in August 2010 showed hypocellular marrow with normoblastic erythropoiesis, normal myeloid maturation and normal megakaryocytes.
What is the cause of thrombocytopenia?

Answer: HIV induced thrombocytopenia
