DIAGNOSTIC DILEMMA
A 5½ years old HIV infected boy had CD4 count of 143 cells/cu mm. He was treated with anti retroviral therapy (ART) consisting of Zidovudine (AZT), Lamivudine (3TC) and Efavirenz (NVP). After 1 month of starting ART, his amylase increased to 899 U/L though child was asymptomatic. USG Abdomen showed bulky pancreas. He had serially elevated serum amylase.

A 4½ month old boy presented with intermittent fever and gradual distension of abdomen since 2 months and self limited oral ulcers 7 days back. On examination, he had splenohepatomegaly. Investigations showed thrombocytopenia, normal liver function tests, negative TORCH titles, reduced megakaryocytes on bone marrow aspiration.

What further tests should be done to diagnose this child ?

Last month's teaching file: A 2 year old girl presented with fever, cough and cold since 4 days. She had received 6 months of antituberculous therapy 1 year back in view of Pulmonary TB in mother and a positive Mantoux test of 10 x 10 mm. On examination, she had crepitations in right inframammary region. Investigations showed right parahilar haziness on Chest X-Ray. She was treated with IV Amoxycillin/Clavulanic acid antibiotic for 5 days but fever persisted. A repeat Mantoux test was 15 x 15 mm. Should this child be treated with Antituberculous therapy?

Answer: There is no need for antituberculous therapy in this child.
