DIAGNOSTIC DILEMMA
A 7 years old boy had left hip arthritis 4 months back for which arthrotomy was done and pus showed mycobacterium tuberculosis (MTB) sensitive to all first line drugs. His hemogram 4 months ago showed lymphopenia (882 cells/cu mm) which again after 4 months of antituberculous therapy was 1364 cells/cu mm. HIV ELISA was negative.

A 10 month old girl presented with not gaining weight, recurrent loose motions and fever since 3 months of age. She has delayed milestones and has achieved only head holding and social smile. On examination, she was wasted (weight = 3.5 kg, length = 60 cm), had oral thrush and loss of subcutaneous fat. There were no lymphadenopathy and tonsils were absent. Systemic examination was normal.

What is the diagnosis?

Last month's teaching file: A 4½ month old boy presented with intermittent fever and gradual distension of abdomen since 2 months and self limited oral ulcers 7 days back. On examination, he had splenohepatomegaly. Investigations showed thrombocytopenia, normal liver function tests, negative TORCH titles, reduced megakaryocytes on bone marrow aspiration. What further tests should be done to diagnose this child ?
