DIAGNOSTIC DILEMMA
New updated Diagnostic Dilemma. Now in an interactive format. Practical cases will discussed with the viewers in an interactive format with a conclusion at the end of it.

A 7 years old HIV infected girl presented with cough for 2 months and fever for 2 months. Father had tuberculosis (TB) 3 years ago and mother had TB 6 years ago. On examination, she had clubbing with generalized non-significant lymphadenopathy and bilateral leathery crepitations in the posterior scapular region. Chest X-Ray showed right lower zone consolidation. She was thus started on antituberculous therapy (ATT). However, she continued to have cough and chest X-Ray showed bronchiectatic changes after 2 months of therapy. She was then started on ART. However she had no response.
What is the cause of her bronchiectasis? How to manage this child?

RESPIRATORY QUIZ
A 10 months old boy presented with severe breathlessness for one day. He was cyanosed. On respiratory system examination, there were bilateral crepitations and wheezing. He was immediately intubated and positive pressure ventilation was started. Chest X ray showed bilateral fluffy shadows. He succumbed to his illness after 24 hours. His throat swab that had been sent for H1N1 PCR was positive. The doctors took oseltamivir prophylaxis for 10 days in view of contact with this patient.
