DIAGNOSTIC DILEMMA

A 1 year 8 months old boy presented with jaundice for 15 days. On examination, he had hepatosplenomegaly. Investigations showed elevated liver enzymes, low albumin, normal viral markers and negative autoimmune markers. Liver biopsy showed micronodular cirrhosis with mallory hyaline disease and copper stain positive suggestive of Indian childhood cirrhosis. Serum copper was high and serum ceruloplasmin was not low. 24 hours urine copper was 269.82. There was no KF ring.
