DIAGNOSTIC DILEMMA
Pediatric Oncall An 11 month old girl resident of Mumbai presented with fever for 15-20 days. She had diarrhea with septic shock, meningitis and anemia 2 months ago for which she was hospitalized and treated with intravenous antibiotics and blood transfusion. On examination, she had pallor, hepatosplenomegaly with generalized lymphadenopathy. Bone Marrow aspiration {Day 48} = LD bodies. This child had never been to an area endemic for Kalaazar though mother had been there 6 months prior to getting pregnant. Child was treated with Inj Amphotericin B to which the fever, pancytopenia responded and hepatosplenomegaly regressed.

How did the child get Kalaazar?

A 4 year old girl presented with recurrent seizures since 1 year of age which are generalized tonic clonic convulsions with clenching occurring once in 2-3 months. Mother had appendicitis in first trimester of pregnancy. A milestone in form of speech is delayed. On examination, the child has microphthalmia, microcornea, low set ears, microcephaly {head circumference = 44 cm}. There was an ejection systolic-murmur in pulmonary area. Other systems are normal.

What is the possible diagnosis?
