DIAGNOSTIC DILEMMA

I am treating a boy who is now 3 year old. At the age of 8 month he presented with poor weight gain and urinary tract infection {UTI}. He was diagnosed as a case of posterior urethral valve {PU valves} after micturating cystourethrogram {MCU} by Pediatric Surgeon. He was undergone cystoscopy 3 times. But even after that his posterior urethra is dilated. He is currently 13 kg with height of 95 cms. I am interested to know about further management and prognosis of the baby.

A 9 years old boy was detected to be HIV infected. His CD4 count was 1078 {30.5%}. At 15½ years of age, his CD4 count was 804 cells/cu mm but HIV viral load was 5,83,300 copies/ml. He is still asymptomatic though his weight was 31.3 kg and height is 144 cm and Tanners Stage 2 of puberty.

Should this child be started on antiretroviral therapy {ART}?

Last month's teaching file: A 12 years old child, a diagnosed case of RHD with MR with MS with mild PH presented with increasing breathlessness since 8 days. On examination, he had irregularly irregular heart rate of around 160/min, respiratory rate of 44/min, blood pressure of 92/60 mm of mercury, cardiomegaly and hepatomegaly. What is the diagnosis?

Answer: Atrial Fibrillation (AF)
